Overview
An open-label extension (OLE) is a follow-up phase of a clinical trial in which both researchers and participants know which treatment is being administered, primarily designed to gather long-term safety and efficacy data.
Clinical trials evaluating new medical treatments typically begin with a randomized, double-blind, placebo-controlled phase. In this "core" phase, neither the patient nor the physician knows whether the active drug or a placebo is being administered. Once this core phase concludes, researchers often invite participants to enter an open-label extension. During the OLE, the blinding is removed. All participants—including those previously assigned to the placebo group—are transitioned to the active experimental treatment. Because the treatment is no longer hidden, the study is termed "open-label." These extensions can last for months or even years, depending on the nature of the disease and the regulatory requirements for the drug.

• While double-blind trials are the gold standard for proving a drug's efficacy, they are often relatively short in duration.
