Dyskinesia refers to a category of movement disorders characterized by involuntary, erratic, and writhing muscle movements. These movements can affect any part of the body, including the face, arms, legs, or trunk, and range from mild tics to severe, exhausting contortions.
While dyskinesia can arise from various neurological conditions, it is most frequently encountered as a side effect of long-term pharmacological treatment. The most common form is levodopa-induced dyskinesia (LID), which affects a significant majority of patients with Parkinson's disease after several years of levodopa therapy. Another well-known variant is tardive dyskinesia, which typically develops following prolonged use of antipsychotic medications that block dopamine receptors in the brain.
The underlying mechanism of dyskinesia is deeply tied to the brain's dopamine system, particularly within the basal ganglia—a network of structures responsible for coordinating smooth, voluntary movement. In Parkinson's disease, dopamine-producing neurons degenerate. Levodopa is administered to replenish this dopamine, effectively alleviating the stiffness and slowness (bradykinesia) associated with the disease. However, over time, the brain's response to fluctuating levels of artificial dopamine becomes erratic. The resulting "peaks" in dopamine concentration can overstimulate the motor cortex, triggering the uncontrolled movements characteristic of dyskinesia.

Managing the condition often requires a complex balancing act by neurologists, who must continuously adjust medication dosages, timing, and formulations to keep patients in a "sweet spot"—where they have enough dopamine to move freely without having so much that it triggers dyskinesia. Advanced interventions, such as deep brain stimulation (DBS) or continuous intestinal infusions of levodopa, are sometimes employed when oral medications fail to maintain this balance.

While mild dyskinesia may be preferred by some patients over the rigid immobility of unmedicated Parkinson's disease, severe dyskinesia can be physically exhausting, socially stigmatizing, and severely detrimental to quality of life. It can interfere with daily activities such as eating, dressing, and walking, increasing the risk of falls and injury. Consequently, mitigating dyskinesia remains a primary focus of ongoing neurological research and drug development.
